NHS Health Checks

The NHS Health Check is a health check-up for adults in England aged 40-74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk. Your NHS Health Check will be done by a healthcare professional. The Health Check typically takes 20 to 30 minutes and includes: measuring your height and weight, measuring your blood pressure, queries regarding your health and advice. Please contact the Practice to arrange an appointment.

Over 75 Health Checks

The Over 75 Health Check is an initiative to improve overall health and well-being in the community.
Its aim is to improve the care and support given to patients aged 75 or over. Evergreen Oak Surgery and Creekmoor Surgery is very supportive of the scheme and will provide Health Checks to improve early diagnosis and health screening
All patients who are over 75 and do not already attend the surgery for an annual review are encouraged to book a health check with the nurse in line with their month of birth.

The nurse will provide assessment and screening of your health and social care issues based on questions and measurements. Following this, you will receive personalised advice and if needed, an onward referral or signposting to appropriate support service.
We would also recommend you read the information contained in the following link Summary Care Records (SCR) - Information for patients and complete the SCR Patient consent preference form and return it back to the surgery.
