Doctors Consultations

We run an routine appointment system at both surgeries. Routine appointments are 10 minutes long and can be booked up to two weeks in advance either through reception or the SystmOnline service.

To book an appointment, please phone 01202 747496 and request the relevant surgery:

• Evergreen Oak Surgery 
• Creekmoor Surgery 

Alternatively, you can book an appointment with your doctor online. To register for the SystmOnline service, please contact a member of the reception staff.

Cancellations/Did Not Attends 

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the appointment to someone else.

We have a high rate of appointments where patients fail to attend and do not call ahead to cancel their appointment. This can lead to a waste of valuable NHS resources and increases our wait for GP appointments, please remember to cancel your appointment by phoning the surgery or replying CANCEL to your confirmation message. After three failure to attend appointments, you will receive a warning letter. 

Urgent Appointments and Telephone Consultations

If you require an urgent same day appointment, you will need to telephone the surgery at 08:30am. You will be booked into an urgent face to face appointment slot, the calls received are handled in order. If there are no urgent appointments left, you may be advised to call back later that day or the following day. 

Urgent telephone appointments are utilised for housebound patients, sickness and diarrhoea, ongoing UTIs for women or an recent surgery that may prevent a patient from coming into the surgery. The doctors aim to take the phone calls as soon as possible, however, please note that there may be a short wait during busy times.

These consultations are limited and should not be used to request repeat prescriptions.

Improved Access

We are working to improve access to appointments for our patients, in particular at evenings and weekends.  If you would like to be offered one of these appointments, please ask.  These appointments may be at a different site; for example, Poole General Hospital.

Nurse Consultations

Nurse appointments are available between 8.30am and 5pm Monday to Friday. Please note the Nurse is available less frequently at Creekmoor Surgery.

To book appointments with the nurse, please contact reception. Please inform reception of the reason for your appointment in order for us to create an appointment of appropriate length.
